Case Manager – Emerging High Risk (EHR) – Virginia (Remote + Field Visits)
Pay Rate: $49.09/hr | Contract Length: 7 Weeks (Possible Extension or Direct Hire)
Location: Remote – must reside in Virginia (Central, Tidewater, South West, or NOVA Regions)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (No weekends)
Travel: 30%+ (Mileage Reimbursed)
Position Overview:
We are seeking five (5) Case Managers based in Virginia to join our Emerging High Risk (EHR) team supporting the Medicaid population. This is a hybrid role combining remote work and field visits to members' homes for face-to-face assessments. Candidates must have excellent computer skills, attention to detail, and be able to manage a fast-paced caseload while documenting accurate contact notes across systems.
Key Responsibilities:
Clinical Assessments: Complete assessments per regulated timelines; identify members who qualify for case management.
Care Planning: Develop, implement, and monitor personalized case management plans in collaboration with healthcare professionals and member support systems.
Member Engagement: Conduct telephonic, virtual, and in-person visits; utilize motivational interviewing to support members.
Care Coordination: Facilitate interdisciplinary care team meetings and integrate behavioral and long-term care services.
Barrier Identification: Identify and address barriers to care, offering resources and interventions as needed.
Documentation: Maintain accurate, timely notes and care plan updates in internal systems.
Compliance: Follow regulatory guidelines and organizational protocols throughout all interactions.
Required Qualifications:
1–3 years of experience in case management, disease management, managed care, or medical/behavioral health settings.
Completion of one of the following:
Accredited LVN/LPN program, or
Bachelor’s or Master’s degree in Social Science, Psychology, Public Health, Social Work, Gerontology, or a related field, or
Graduation from an Accredited School of Nursing (BSN preferred).
Must have an active, unrestricted license if the field of study requires licensure.
Valid driver's license with a good driving record and access to reliable transportation.
High-speed internet required for remote work.

Proficiency with multi-system navigation and electronic documentation.
Additional Information:
This is a 7-week contract through June 30th, with potential for extension or direct hire depending on contract outcomes.
Candidates must be comfortable with 30%+ travel to complete home visits.
