Remote RN – Discharge Planner (CA License Required)
Pay Rate: $45.99/hour
Contract Length: 3 Months (Potential for Extension or Direct Hire)
Location: Remote (Candidate must reside in California and hold an active CA RN license)
Schedule: Monday – Friday, 8:30 AM – 5:30 PM PST

Our Client is seeking an experienced Registered Nurse to support discharge planning operations for California Health Plan members. This role focuses on ensuring safe and efficient transitions from hospitals and other care settings to lower levels of care.
Key Responsibilities:
- Collaborate with California hospitals, SNFs, LTACs, ARUs, and custodial care facilities to discharge members safely to appropriate care settings.
- Utilize MCG criteria to support medical necessity and discharge decisions.
- Coordinate discharge planning in alignment with Medi-Cal post-acute care benefits and facility contract requirements.
- Communicate compassionately and professionally with providers and members to ensure seamless transitions of care.
- Maintain a high level of accountability to ensure timely, appropriate discharges.
Must-Have Qualifications:
- Minimum 5 years of clinical experience in acute care (Med-Surg, Telemetry, ICU, or ER).
- At least 2 years of experience as a hospital-based discharge planner, with demonstrated expertise in transitions to SNFs, LTACs, ARUs, and custodial care.
- Strong knowledge of Medi-Cal post-acute care benefits.
- Proficiency in MCG inpatient criteria review and interpretation.
- Experience reviewing facility contracts related to post-acute levels of care.
- Must be currently licensed as a Registered Nurse in California and reside in California.
Preferred Experience:
- Managed care experience strongly preferred.
- Excellent communication, organization, and coordination skills.
- Demonstrated ability to work independently in a remote environment.
Equipment Note: Candidates will require dual monitors and a docking station to perform duties effectively.
