We have a client, a major Hospice/Home Health organization, that is looking for an Acute Care RN in Independence, MO!

Will accept new RN's!

Salary: $75,0000- $85,000.00 per year

Mileage: .58 cents per mile reimbursement/15-30-mile radius from Independence, MO

Schedule: Monday-Friday 8:00am - 5:00pm – (Friday On Call Rotation) / 1 day in the office throughout the work week

Acute Care RN Job Responsibilities:

• Complete an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness.
• Responsible for a 15-30 patient case load
• Assess and evaluate patient’s status by initiating plan of care, regularly re-evaluating patient, and family/caregiver needs, and initiating the plan of care and making necessary revisions as patient status and needs change.
• Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and the family in the planning process.
• Initiate the appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
• Provide health care instructions to the patient as appropriate per assessment and plan and assist the patient with the activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care.
• Ensure that arrangements for equipment and other necessary items and services are available.

Acute Care RN Job Requirements:

• Active/unrestricted RN License in the state of Missouri or Compact License.
• 2+ years of experience in a Hospice/Home Health setting in a Case Management role. (Preferred)
• Active BLS
• Must have a valid Driver's License and can provide proof of insurance.
